When you submit your application we will need copies of:

• Social Security Cards for everyone in the household. All Applications
• Birth Certificates for everyone in the household. All Applications
• Driver’s License for all family members who have one.
• Verification of Income- Social Security Benefit letter & all other income
• If you receive Food Stamps Please provide documentation with the amount
• Most recent Bank Statement
• Most recent tax return if you have one
• Pharmacy Printout. For all Elderly and Disabled Applicants
• Medical Statements – Showing balance due and/or out-of-pocket expenses. For all Elderly and Disabled Applicants
• Receipts for over-the-counter Medications– With a letter from your physician stating that you should be taking these medicines. For all Elderly and Disabled Applicants
• STUDENTS- Class schedule & financial aid letter
